Our personalized placement process is all about making sure we identify the RIGHT placement, in the RIGHT location, for the RIGHT price. No two placements are alike, and as such, personalizing the process for YOUR needs is paramount in what we do. The placement process generally follows this process
As soon as a request for help is made, you will be contacted by your assigned Placement Angel Specialist. This Specialist will be an expert in your particular area, as placement options vary greatly by state and even zip code sometimes. Depending upon the urgency of your situation, your Specialist will set up a time to perform a full assessment, generally in-person, or by phone if the timing requires expediency. The goal of every assessment is to gather ALL of the information pertinent to the placement at hand.
Once your placement specialist has all the information needed to understand the needs/wants of your placement, they will review the top 3 options with you to familiarize you with those and plan the next step. PA specialists narrow the options down to 3 as we have seen that adding more to the mix can be quite confusing. However, if none of the 3 initial options presented are the perfect fit, they will present you with additional options.
Your PA Specialist and you will coordinate a convenient time to tour the options presented. These guided tours will give you the opportunity to see the options for yourself and meet the residents, caregivers, other staff, etc. Your PA specialist will also be there to help you get answers to any questions you might have about that particula option.
The decision of where to choose is ultimately yours. However, your PA specialist will be there to give advice in the process, especially in trying to anticipate what things might be important in the future. In some cases, all the options might seem like the right fit. This is a good “problem” to have and your specialist will help guide you through potential pros and cons of each.
Once a selection has been made, your PA specialist will coordinate between the place chosen and the discharging facility or other medical professionals involved. This may include helping coordinate paperwork, transportation needs, move-in requirements, etc. The goal is to make the move as stress-free as possible for you and your loved one(s).
Shortly after move-in, your PA specialist will follow-up with you to make sure that all expectations and needs are being met. They will remain ON YOUR TEAM from that point forward and will continue to advocate for you.
Although it is the intention that every move-in NOT need adjustments or change, it does happen, especially when the needs of the resident change substantially. If that is the case, your PA specialist will help you with the next transition necessary.
